Washers with square holes do go on top. The offset on them is what keeps the carriage bolt from turning in the wood. The washers underneath have round holes.

Prowbar
Washers with square holes go on top on carriage mounting bolts. Offset of washer keeps carriage bolt from turning when mounting bolts are tightened.

Thanks for the help. Makes sense that they go on top. Where are they located on a fleetside? (the washers) Looking at pictures I cannot spot them. I figured each and every bolt passed through the bedstrip with a square hole in it. Or are there exceptions?

Bolts with offset washers mount bed to frame of truck. I don't have fleet side so I can't give you measurements. The bolts for these washers are much longer and 3/8" diameter instead of the 1/4" rail bolts. If you bought a bolt kit, these bolts are probably 4" long.
